The driver for a package delivery company is in trouble after being involved in a two-vehicle collision that injured himself and three others in Baldwin County last week.

The crash happened about 9:15 a.m. on Wednesday, June 25 at the intersection of Log Cabin and Airport roads, local and state authorities said.

The delivery truck driver, identified as Travis Lymorn Carithers, 35 of Mableton, since has been charged with driving under the influence, failure to maintain lane and tire requirements, according to a wreck report filed by Sgt. First Class Scott Harrell, commander of the Georgia State Patrol post in Milledgeville.

Carithers and the three workers in the other vehicle were all injured. They were treated at the scene by firefighters with Baldwin County Fire Rescue and Milledgeville Fire Rescue Services and personnel with Atrium Health Navicent Emergency Medical Services.

Carithers and two of the workers with the Pamlico Pool Company in Milledgeville were all taken to the emergency department of Atrium Health Navicent Baldwin hospital in Milledgeville. The pool company employees were identified as Efren Guitirrez. 34; and Alexis Hernandez, 26, both of Milledgeville.

The more serious of those injured was the driver of the Pamlico Pool Company truck, identified as Gerardo Cruz, 37, also of Milledgeville.

He was taken to Atrium Health Navicent – The Medical Center in Macon in what authorities at the scene said was critical condition.

Sgt. Harrell said Carithers was traveling east on Log Cabin Road, just west of Airport Rd. in a 2018 Mercedes Benz Sprinter van.

Cruz, meanwhile, was traveling west along Log Cabin Road, just west of Airport Rd. in a 2012 Ford DRW Super Duty truck.

The investigating state trooper said Carithers failed to maintain his lane and crossed over the center line divider. The left front of Carithers’ van struck the left front of Cruz’s work truck.

“The area of impact was approximately six-feet north of the center line divider in the westbound lane, as determined by debris and gouge marks,” Harrell said in his report.

After impact, the van being driven by Carithers yawed to the right and overturned on its left side.

Cruz’s truck came to a final rest near the area of impact in the westbound lane, Harrell said.

The trooper said Carithers stated he did not know how the crash happened.

“He stated he was not on his phone or otherwise distracted prior to the crash,” Harrell said. “The investigation revealed driver one (Carithers) was impaired.”

A post-crash investigation of the delivery van also revealed both rear tires had less than 2/32nds of tread depth, Harrell said.
